Procedures

  1. The applicant visits Health Professions Licensing Department and submits the request as per the designated form along with the required documents.
  2. The documents are verified and the application is approved.
  3. The Committee will then review the enclosed documents, and schedule the date of the field inspection of the center.
  4. In case the fittings of the center are in compliance with Law no. (108) for the year 2016, the applicant pays the required fees.
  5. The applicant receives the approval letter signed by the Minister of Health for licensing and direct action.

Required Documents

  • A survey plan by a licensed surveyor or an engineering office showing the area of the center.
  • A Professional License as a prosthesis technician.
  • A five-year experience certificate by the Ministry of Health/ Jordanian Royal Medical Services/ Rehabilitation colleges/ any subsidiary hospital of official universities.
  • A certificate of proof that the applicant is in not working for any other party.
  • The area of the center must be no less than (120) meter.  
  • A valid organization plan of the site.
  • A copy of the commercial register issued by the Ministry of Industry and Trade.
  • An ownership deed or a lease contract.
